This issue relating to InfoPath form services and SPWorkflowTaskProperties.ExtendedProperties, was posted and answered by MarcusF (InfoPath Dev Forums):

"I solved it! The *name* of the data connection must be ItemMetadata.xml as well as the *file name* being ItemMetadata.xml."
